PP076-MON INFLUENCE OF TWO TYPES OF LIPID EMULSIONS ON THE LIVER FUNCTIONAL STATUS OF PATIENTS WITH COLORECTAL LIVER METASTASIS RECEIVING CHEMOTHERAPY. A PROSPECTIVE RANDOMIZED STUDYстатья

Аннотация:Rationale: Conducting antitumor chemotherapy accompanied by the increasing of protein-energetic insufficiency as a consequence of obligate amplification catabolic processes (cytotoxic effect of chemotherapy) and development of gastrointestinal toxicity (GT), which requires total parenteral nutrition (TPN). The aim of this study is to assess the effects of PN-mixtures “all in one” with different lipid emulsions (LE) on hepatic integrity.Methods: Forty patients (M:F=18:22, mean age 56) with colorectal liver metastasis receiving chemotherapy (Oxaliplatin, Capecitabine) who developed Stage III-IV of GT in the course of the treatment were recruited and were randomized into 2 groups. The patients of group 1 (G1, n=20) received TPN with medium-chain and long-chain triglycerides (Nutriflex 48/150 Lipid, soybean oil). The patients of group 2 (G2, n=20) received TPN based on olive oil and soybean oil (Oliclinomel-7). The total energy intake per day was 25 kcal/kg body weight (BW), amino acids:1 g/kg BW, glucose:3,3 g/kg BW, fat:1 g/kg BW. TPN was given for 7 days. Aspartate aminotransferase, alanin aminotransferase, alkaline phosphatase, gamma-glutamyl transferase (GGT), concentration of total bilirubin and clinical data were measured before the start of TPN and after the administration. The significance of differences was assessed by Student’s t-test and Chi-square test.Results: At the baseline anthropometric and biochemical parameters in both groups did not differ. There were no significant differences in the concentrations of biochemical markers of hepatic failure and clinical data. Both groups showed evident increase of GGT activity (G1: 88,2±18,7 vs 141±20,1 U/l, G2: 86,5±16,8 vs 134±22,1 U/l).Conclusion: No significant differences in the administration of TPN mixtures with different LE have been revealed.
