Аннотация:Background: The purpose of this study is to identify the cognitive processes in two groups of patients with Parkinson's disease with different levels of disturbances. The cognitive processes in patients were compared with two control groups of different age. We focused on the mechanism of anticipation as a special type of memory and the mechanism of semantic categorization. The experiment included the identification of semantic categories during the visual presentation of words belonging to two domains: ‘animals’ and ‘objects’. Methods: Our method of ‘Microstructural analysis of oscillatory brain activity’ (Danilova, 2002) based on the pacemaker hypothesis of the rhythmogenesis was used to study the cognitive disturbances in patients with Parkinson's disease (PD). The method includes the multichannel EEG recording and calculation of equivalent current dipoles for narrowband frequency-selective generators extracted from the evoked potentials. The two groups of patients were compared with two control groups. The first group of patients (11 subjects: 4 males and 7 females, 64-79 years old) with 1-3 Hoehn-Yahr stage of disease was medicated with L-dopa alone. The second group of patients with more early stage of PD (11 people: 6 males and 5 females, 47-68 years old) with 1-2 Hoehn-Yahr stage of disease was medicated with agonists of dopamine receptors. Every both control groups are consisted of 10 people (5 males and 5 females) and differed only by the age (the young group 22-23 years old and aged group 50-60 years old). Results: Reactions of patients are differed from control groups when they recognized the category of ‘animals’. The both groups of patients demonstrated the increasing of latent periods of motor responses and number of errors with the suppression of frequency-selective theta generators activity. The patients receiving L-dopa showed greater preservation of the theta rhythm and the best results for identification of categories than the patients who has received only the agonists of dopamine receptors despite the fact that they were on the earlier stage of the disease. Disturbances of the mechanism of identification of ‘animal’ category were also revealed before stimulus at anticipation stage in patients. Conclusion. The disturbances of the mechanism of anticipation during the experiment with the identification of the category of ‘animals’ can be used as an early marker of Parkinson's disease.
