Проверка гипотезы о том, что коррекция физиологического состояния митохондрий эндотелия митохондриально-направленными соединениями семейства SkQ будет предотвращать избыточное сосудистое воспаление и может быть эффективной стратегией для терапии осложнений, связанных с гипертрофированным системным воспалительным ответом.
At the beginning of the COVID-19 pandemic, it was found that bilateral pneumonia was the most important clinical manifestation of SARS-CoV-2 infection, and acute respiratory distress syndrome (ARDS) was the main mortality facto. Now it is generally accepted that the complication of the disease is associated not only with direct viral damage, but to a greater extent with immune-mediated tissue damage caused by SARS-CoV-2. In patients with severe and critical forms of COVID-19, two distinctive features were noted: a progressive increase in inflammation and an unusually manifested tendency to hypercoagulation. The experience of corticosteroid treatment of COVID-19 showed serious limitations on their applicability, primarily associated with the general suppression of specific immunity and an increased likelihood of developing secondary bacterial pneumonia. COVID-19 treatment protocols include inflammatory cytokine antagonists such as TNF inhibitors, tocilizumab (IL-6 inhibitor), or anakinra (IL-1 receptor antagonist). However, their application is associated with tremendous cost and has a number of limitations due to their narrow action regarding only one of the many inflammatory cytokines. Inhibition of endothelial activation by NFkB activation suppression in endothelium has shown promising results in animal models of sepsis and systemic inflammatory response syndrome. Targeting the endothelium is a potentially effective strategy for the treatment of pathologies associated with a hypertrophic systemic inflammatory response, including those concomitant with a viral infection. The aims of proposed project are to prevent ARDS caused by the SARS-CoV-2 virus with the application of the mitochondria-directed compound SkQ1, which simultaneously has the properties of an antioxidant and a “mild” uncoupler of oxidative phosphorylation. Anti-inflammatory effect of SkQ1 in various animal and cell models is shown in studies conducted by our group in recent years. SkQ1 also prevented a decrease in body temperature and animal death in the model of acute uncontrolled systemic inflammation caused by the administration of a lethal dose of TNF to mice in the tail vein. It is important to note that recently it was shown in a preliminary experiment that SkQ1 prevents the death of mice caused by the introduction of the human influenza virus strain H5N1 when administered intraperitoneal. One of the main targets of the SkQ1 therapeutic effect is the vascular endothelium. The anti-inflammatory mechanism of SkQ1 was determined by ability of its nanomolar concentrations to inhibit the activation of the transcription factor NF-κB which regulates the expression of pro-inflammatory inflammatory cytokines, adhesion molecules, and extracellular matrix metalloprotease 9 (MMP9) in the endothelium. The ability of SkQ1 to protect vascular endothelium from excessive activation and damage by inflammatory mediators allows it to be considered as a potentially effective drug for the treatment of ARDS in viral infections. Several objectives are set to assess the possibility of SkQ1 application in the treatment of ARDS in patients with COVID-19. These objectives are: (1) to study the potential effect of SkQ1 on the activation of human respiratory and lung epithelial cells by viral proteins, (2) to study the potential effect of SkQ1 on impaired barrier functions, activation and death of the endothelium of the respiratory tract caused by viral proteins, angiotensin 2, as well as growth medium from pulmonary epithelial cells activated by viral proteins, (3) in case of positive results being obtained from the experiments case with SkQ1 in cellular models, it is planned to hold SkQ1 tests on transgenic mice expressing human APF2. In this model, intranasal injection of SARS-CoV-2 causes a disease that includes weight loss and interstitial pneumonia.
Мы ожидаем, что будет показана возможность применения SkQ1 для предотвращения и лечения связанных с сосудистым воспалением осложнений COVID19. На первом этапе проекта мы ожидаем, что будет показана эффективность использования SkQ1 для предотвращения активации эндотелиальных клеток легких под действием ангиотензина 2 и IL-6 in vitro. На первом этапе будут также получены и очищены вирусные белки и псевдовирус, несущий S-белок на капсиде и будут подобраны условия, в которых они будут вызывать активацию эндотелиальных и эпителиальных клеток легкого и поставлены предварительные эксперименты, позволяющие оценить эффективность SkQ1.
В исследованиях, проведенных нами в последние годы, показано выраженное противовоспалительное действие SkQ1 в различных животных и клеточных моделях (Chelombitko et al., 2017; Demianenko et al., 2010; Demyanenko et al., 2015, 2017; Galkin et al., 2016, 2014; Romaschenko et al., 2015; Vorobjeva et al., 2017; Zakharova, Pletjushkina, Galkin, et al., 2017; Zakharova, Pletjushkina, Zinovkin, Popova, & Chernyak, 2017; Zinovkin et al., 2014). В том числе в модели острого неконтролируемого системного воспаления, вызванного введением мышам в хвостовую вену летальной дозы TNF SkQ1 предотвращал снижение температуры тела и гибель животных (Zakharova, Pletjushkina, Galkin, et al., 2017). SkQ1 подавлял экспрессию ряда про-воспалительных маркеров в аортах старых мышей (Zinovkin et al., 2014) и мышей с острым системным воспалением (Zakharova, Pletjushkina, Galkin, et al., 2017). In vitro в модели активации эндотелия нетоксичными концентрациями TNF SkQ1 подавлял способность эндотелия связывать лейкоциты (Romaschenko et al., 2015; Zinovkin et al., 2014), а также снижал проницаемость монослоя эндотелия для высокомолекулярных молекул (Demyanenko et al., 2015; Zakharova, Pletjushkina, Galkin, et al., 2017). Механизм противовоспалительного действия SkQ1 в обеих моделях определялся его способностью в наномолярных концентрациях ингибировать активацию транскрипционного фактора NF-κB, который регулирует экспрессию про-воспалительных цитокинов воспаления, молекул адгезии и металлопротеазы внеклеточного матрикса 9 (ММР9) в эндотелии (Zakharova, Pletjushkina, Galkin, et al., 2017; Zinovkin et al., 2014). SkQ1 также подавлял апоптоз эндотелиальных клеток, вызванный высокой концентрацией TNF (Galkin et al., 2014) и расщепление белка межклеточных котактов-катенина в результате подпороговой активации каспаз (Galkin et al., 2016).
